Levelling Up Fund: Pro Forma 7 Senior Responsible Owner Declarations

All bids are required to submit a signed declaration from their Senior Responsible Owner (via this pro forma) alongside their application. This is an eligibility requirement and if it is not met the bid will not proceed to assessment.

Copy the pro forma into a blank editable document. The lead applicant should fill in the blank sections, insert a signature from the Senior Responsible Owner, and then upload to the online portal with the application by the submission date. This must not be submitted in any other way.
